SUPPORT ROOTED IN LIVED EXPERIENCE

Recovery can feel different when someone truly understands the journey.

At Legacy of Hope Care, we understand that recovery is a personal journey, and sometimes having someone who truly understands can make a meaningful difference.

Our Peer Support Services connect individuals with trained North Carolina Certified Peer Support Specialists (CPSS) who use their own lived experience to provide encouragement, hope, education, advocacy, and practical support.

North Carolina Certified Peer Support Specialists are individuals with lived experience who support others who may benefit from that experience. Their role includes building relationships based on trust, respect, empathy, mutual experience, and individual choice.

Our Peer Support Specialists work with individuals to identify personal strengths, develop achievable goals, increase self-advocacy, strengthen recovery skills, and become more connected to their communities.

One-on-One Peer Support

Individualized support based on the person’s goals, needs, strengths, and recovery journey.

Goal Setting & Accountability

Helping individuals identify achievable goals, develop action steps, and stay connected to the goals they have identified for themselves.

Recovery Education

Sharing practical information and lived experience that can help individuals better understand recovery, wellness, available supports, and self-advocacy.

Community Resource Navigation

Helping individuals connect with appropriate resources related to behavioral health, housing, employment, education, transportation, relationships, and other areas of need.

Self-Advocacy

Helping individuals develop the confidence and skills to communicate their needs, make informed choices, and participate actively in their care.

Social & Community Connection

Encouraging healthy relationships and meaningful participation in community activities and supports.

Ongoing Encouragement

Providing consistent, recovery-focused encouragement while respecting the individual’s choices, values, culture, and personal goals.

WHO MAY BENEFIT?

Structured peer-based support for eligible adults working toward recovery.

Peer Support Services may benefit individuals who are working toward recovery from a mental health condition and/or substance use disorder and who can benefit from structured peer-based support.

North Carolina Medicaid’s Peer Support Services policy describes the service for eligible adult Medicaid beneficiaries diagnosed with a mental health or substance use disorder. Medical necessity, service authorization, and other program requirements apply.

NORTH CAROLINA CERTIFIED PEER SUPPORT SPECIALISTS

Lived experience combined with professional peer-support training.

Legacy of Hope Care’s Peer Support program can utilize North Carolina Certified Peer Support Specialists who meet applicable state requirements.

North Carolina’s certification program requires specific eligibility, training, documentation, and certification requirements. Current NCDHHS information states that applicants must have lived experience in recovery from a significant mental illness and/or substance use disorder, a high school diploma or equivalent, approved training, additional training including ethics and peer-support boundaries, references, and other certification requirements.

Certified Peer Support Specialists bring something unique to the care team: lived experience combined with professional peer-support training. Their experience can help create connection, hope, trust, and understanding.

PERSON-CENTERED SUPPORT

Every person’s recovery journey is different.

Legacy of Hope Care uses a person-centered approach that recognizes each individual’s strengths, goals, preferences, culture, values, lived experiences, support system, and community needs.

Our Peer Support Specialists meet individuals where they are and provide support without judgment.

The goal is not to tell someone what their recovery should look like. The goal is to walk alongside them, listen, encourage them, and help them work toward the goals they have identified for themselves.
